Unintended consequences of long-sleeved gowns in a critical care setting during the COVID-19 pandemic

新型コロナウイルス感染症（COVID-19）パンデミックのピーク時に、手指衛生監査により人工呼吸器を装着された COVID-19 患者を収容した 12 床の救命救急病棟において手指衛生遵守率が低いことが示された。そこでは、スタッフは個人防護具（PPE）として、Public Health England の推奨に従って長袖ガウンの患者ケア時における使用などを行っていた。無菌部位から腸内グラム陰性菌が分離された患者数の増加に伴って、3 件の中心静脈カテーテル（CVC）関連感染症のクラスターも発生した。患者周囲表面および高頻度接触部位の環境サンプリングから、COVID-19 救命救急病棟では面積の 11.5％が腸内グラム陰性菌により汚染されていたのに対し、COVID-19 一般病棟および非 COVID-19 一般病棟の汚染率はそれぞれ 2.6％および 2.7％であったことが示された。リスク評価を行った後、病院の方針を変更して、長袖ガウンに代えて半袖ガウンを使用することとした。救命救急病棟において、次亜塩素酸塩ベースの消毒薬を用いた清掃の強化と、8 日後に再サンプリングを実施した。再サンプリングの結果、救命救急病棟ではグラム陰性菌は分離されなかった。このようなPPEの変更後、手指衛生遵守率は、ベースラインの基準レベルに回復し、新たな CVC 関連感染症は同定されなかった。スタッフは、半袖ガウンのほうを好むと報告した。現在、パンデミックインフルエンザに対して推奨されている PPE（呼吸器防護具＋標準 PPE）以外の使用が、医療従事者においてSARS-CoV-2 に対する防御効果を高めるというエビデンスは存在しない。長袖ガウンは、効果的な手指衛生を実施している医療従事者を防御する。医療従事者が十分に防御されることは不可欠である一方、患者を感染症の危険から防御することも同様に重要である。PPE に関連するリスクを確立して、現行のガイダンスを見直すための情報を得るためには、さらなる研究が必要である。

本報告より長袖ガウンは手袋を外した後の医療従事者が手指衛生をする際の障壁になることがわかる。SARS-CoV-2 への曝露予防には長袖ガウンが一般的に推奨されているが、他の感染症予防の観点も含めて、半袖エプロンの使用も総合的に考える必要がある。是非、本論文を一読されたい。
