Spontaneous decolonization during hospitalization in intensive care unit patients colonized by extended-spectrum beta-lactamase-producing Enterobacterales

本研究の目的は、基質特異性拡張型βラクタマーゼ産生腸内細菌目細菌（ESBL-E）を保菌する集中治療室患者を対象とした週 1 回の ESBL-E 直腸保菌スクリーニングを継続することの付加価値を評価するために、これらの患者における自然除菌率を分析することである。12 年間にわたり患者 20,846 例から週 1 回採取した計 49,468 の直腸スクリーニング検体を含めた。少なくとも 3 回連続で検体が陰性を示し、入院の終わりに除菌と判定されたのは、 ESBL-E 保菌者 4,280 例中 109 例（2.5％）にすぎなかった。全体で、ESBL-E 保菌者として既に特定された患者の 7,957 検体（16.1％）の検査が依頼された。ESBL-E 保菌の陽性所見が得られた後に、不要な週 1 回のスクリーニングを避けることで、看護や検査室での作業の負担を減らせるであろう。

監訳者コメント：
ESBL 産生腸内細菌目細菌の保菌検査を行っている施設はその方法を見直す上で一読すると良いだろう。保菌者に対して毎週スクリーニングするのは無駄が多いようだ。
