Surveillance of bloodstream infections in intensive care units in England, May 2016–April 2017: epidemiology and ecology

背景
集中治療室（ICU）の患者における血流感染症（BSI）は、罹病率・死亡率・経済的損失の増加と関連する。多くの BSI が中心静脈カテーテル（CVC）と関連している。英国の ICU における BSI サーベイランスに着手するために、Infection in Critical Care Quality Improvement Programme が構築された。
方法
あらゆる血液培養陽性例、入院日数、CVC 留置日数について収集するために、2016 年 5 月 1 日にウェブベースのデータ収集システムを立ち上げた。英国国民保健サービス（National Health Service；NHS）トラストに、サーベイランスプログラムへの参加を依頼した。データを、Public Health England により保存されている抗菌薬耐性データセットおよび死亡率データとリンクさせた。
結果
2016 年 5 月 1 日から 2017 年 4 月 30 日にかけて、NHS トラストの 147 施設のうち57 施設の ICU 84 室（成人 ICU 72 室、小児 ICU 7 室、新生児 ICU 5 室）がデータを提供した。全体で 1,474 件の血液培養陽性例が報告され、もっともよく報告された微生物は、コアグラーゼ陰性ブドウ球菌（staphylococci）、大腸菌（Escherichia coli）、黄色ブドウ球菌（Staphylococcus aureus）、エンテロコッカス・フェシウム（Enterococcus faecium）であった。成人 ICU、小児 ICU、新生児 ICUそれぞれで、BSI の発生率は 1,000 患者ベッド日あたり 5.7、1.5、1.3 で、ICU 関連 CVC-BSI の発生率は 1,000 ICU-CVC 日あたり 2.3、1.0、1.5 であった。ICU のタイプ毎で、 BSI および CVC-BSI の発生率に大きな差があり、とくに成人 ICU で顕著であった（1,000 患者ベッド日あたり 0 ～ 44.0、1,000 ICU-CVC 日あたり 0 ～ 18.3）。
結論
ICU 関連 CVC-BSI の全発生率は、3 つの年齢層で 1,000 ICU-CVC 日あたり 2.5 未満であったが、ICU 間で大きな差が認められた。これによる改善の機会を特定するために、全国的な標準化サーベイランスシステムの重要性が強調される。データリンケージによって、耐性パターンおよび患者転帰に関する臨床的に重要な情報が、追加費用なしで参加トラストに提供された。
