医療環境における抗菌剤コーティングによる耐性の淘汰

抗菌性接触面は、現行の衛生手技をサポートするため、また、高まる抗菌剤の耐性の脅威への対処を補助するために医療環境に導入されている。とはいえ、抗菌性接触面によって、抗菌剤耐性の出現および拡散を助長しうる選択圧の可能性が懸念されている。本レビューでは、新規突然変異および遺伝子水平伝播による進化など異なるプロセスならびに抗菌性表面に散在する自然耐性菌の種選別により生じる抗菌性表面の耐性と関連するリスクを示す研究に焦点を置いた。また、実用化されている銅や銀、および抗菌ペプチドの有望な特性から、銅、銀、抗菌ペプチドを使用した抗菌性表面に注目した。入手可能なデータから、金属によりもたらされる交差耐性・共耐性および抗菌剤耐性の特性によって起こる耐性選択と、それに続く耐性剤増加の可能性が示された。しかしながら、医療関連環境での抗菌性接触面に対する耐性の出現について報告した橋渡し研究はまれであり、この環境において抗菌性表面が耐性選択を誘導するか否か、また、いかに誘導するかを評価する必要がある。このような研究では、コーティングに含まれる抗菌剤濃度、表面上のバイオフィルム発生、乾燥表面環境と関連する湿度など多数の変数について検討する必要があると考えられる。抗菌コーティングの有効性に関する現場試験では、その使用と関連する耐性選択のリスクをルーチンに評価すべきである。

監訳者コメント:
抗菌剤処理を施した環境表面の効果がどれだけあるのかについては十分な検証が必要である。乾性バイオフィルム対策としても効果的なのかどうかなどについても検証が必要である。
