流行中の OXA-48 産生肺炎桿菌(Klebsiella pneumoniae)のスクリーニングのための簡単で信頼性の高いワークフローの前向き評価

背景
カルバペネマーゼ産生腸内細菌目細菌(CPE)は公衆衛生に対する深刻な脅威の 1 つである。臨床微生物検査室は CPE のスクリーニングと確認のための効果的なプロトコールを必要としている。
目的
OXA-48 が流行中の病院 1 施設において、カルバペネマーゼ産生肺炎桿菌(Klebsiella pneumoniae)のスクリーニングのアルゴリズムを前向きに評価すること。
方法
本アルゴリズムは、ルーチンの自動抗菌薬感受性検査の純度検査としての役割も果たすエルタペネムとテモシリンを用いたディスク拡散アッセイに基づいた。エルタペネムに対する最小発育阻止濃度> 0.5 mg/L または阻止円直径< 25 mm(判定基準 1)、ならびにテモシリンに対する阻止円直径< 12 mm(判定基準 2)のすべての分離株を、OXA-48 側方流動免疫クロマトグラフィーアッセイおよび VIM、KPC、OXA-48 を標的とするマルチプレックス PCR により順次検査した。どちらの検査も陽性でない場合、modified Hodge test または CARBA NP test を使用した。
結果
2 年で 2,487 の肺炎桿菌を提示したアルゴリズムで評価し、378(15.20%)が両方の判定基準を満たした。これらのうち 98.68%(378 中 373)は OXA-48 産生株として、あるいは長期間同じ耐性表現型を維持した以前の CPE の分離株を有する患者由来として確認された。残りの 3 つの肺炎桿菌は VIM 産生株であった。判定基準 1 と 2 を満たすにもかかわらずカルバペネマーゼを産生しなかったのは、378 の分離株のうち 2 つ(0.53%)のみであった。
結論
記述したアルゴリズムは CPE の検出に関して最も感受性の高いカルバペネムと OXA-48 の検出に関して特異性の高いテモシリンのカットオフを組み合わせた。本アルゴリズムは臨床微生物検査室のルーチンのワークフローに適用する上で、信頼性が高く簡単であり、CPE 分離株の迅速な検出を可能にすることにより、感染制御策と標的を定めた抗菌薬レジメンの迅速な実施が可能となる。
