外科・内科患者における静脈内および尿路カテーテルの不適切な使用の発生率とリスク因子

背景
我々は以前に医療関連感染を防ぐ目的で、内科病棟において静脈内および尿路カテーテルの不適切な使用を減らすために RICAT(Reduction of Inappropriate use of intravenous and urinary CATheters)スタディを実施した。
目的
外科病棟と内科病棟を比較すること、ならびにカテーテルの不適切な使用のリスク因子を特定すること。
方法
2017 年 10 月から 2018 年 5 月に、オランダの大学病院 2 施設の外科病棟において横断研究を実施した。患者を 7 か月間隔週で前向きに観察した。不適切な使用は RICAT スタディの外科以外の病棟と比較した。
結果
合計で 409 例の外科患者が含まれ、1,781 例の内科患者と比較された。373 例の外科患者の 425 本の末梢静脈カテーテルのうち不適切な使用は 36 本(8.5%)で発生し、1,665 例の内科患者では 1,747 本の末梢静脈カテーテルのうち 400本(22.9%)であることと比較すると、差は 14.4%(95%信頼区間[CI]11.1 ~ 17.8、P < 0.001)であった。134 例の外科患者のうち尿路カテーテルの不適切な使用は 14 例(10.4%)で発生し、324 例の内科患者のうち 105例(32.4%)であることと比較すると、差は 22.0%(95%CI 14.7 ~ 29.2、P < 0.001)であった。大学病院 2 施設のサブグループ解析によりこれらの差が裏付けられた。末梢静脈カテーテルの不適切な使用の主なリスク因子は内科病棟への入院(オッズ比[OR]3.50、95%CI 2.15 ~ 5.69)であり、これは尿路カテーテルでの主なリスク因子の 1 つでもあった(OR 2.75、95%CI 1.36 ~ 5.55)。
結論
カテーテルの不適切な使用は外科病棟と比較して内科病棟でより発生しやすい。医療関連感染を減少させるための予防戦略は、不適切な使用の発生率が高い場所に第一に重点を置くべきである。

カテーテルの不適切な使用が感染のリスクになることは疑う余地がないと思われる。原因分析とそれに基づいた対策、その評価についての続報が気になるところである。
