Prevention of nosocomial legionellosis by best water management: comparison of three decontamination methods

背景
2000 年以降、国民保健サービスでは、病院の給水システムにおけるレジオネラ属菌評価の国際ガイドラインを採用している。レジオネラ属菌による水汚染の管理は、依然として院内感染症予防の最適な方法に関する研究対象である。
目的
コロニー形成単位（cfu）数および病院獲得レジオネラ症の症例数のモニタリングにより、3 種類の汚染除去方法を比較すること。副次的目的は、配水パイプに対する予防策の長期効果を評価することとした。
方法
サンプリング対象となる高リスク箇所の選択、ならびに 19 年間にわたり以下の 3 種類の消毒法について評価を行うためのプロトコールを作成した：超塩素処理（hyperchlorination）および熱衝撃（期間A、2000 年から 2005 年）、銅-銀イオン化（期間B、2006 年から 2010 年）、ならびにプレフィルタリングとフィルタリングの統合、パイプ保護製品、および二酸化塩素（ClO₂）を用いた遠隔制御（期間C、2011 年から 2018 年）。
結果
衝撃消毒および超塩素処理の使用により、実施後速やかに汚染レベルの低下が得られたが、2 か月後には元のレベルに上昇した。銅-銀イオン化および ClO2 消毒では両方とも、汚染レベルの安定的かつ持続的な低下が示された。上記の 3 つの期間を通じて、レジオネラ属菌による症例 6 例が期間 A に、6 例が期間 B に、そして 3 例 が期間 C に発生した。配水管のダメージに関しては、効果的な銅-銀レベルは腐食および石灰化を引き起こした。
結論
銅-銀イオン化および ClO₂ はいずれも、レジオネラ属菌による汚染を適切に制御した。ClO₂ は陽性箇所の数を有意に減少させ（P＜0.001）、配水管にダメージを与えなかった。
