開頭術の術後肺炎:発生率、リスク因子、ノモグラムによる予測

背景
術後肺炎は手術患者の合併症として 3 番目に多い。しかしながら、脳神経外科においてもっとも頻度の高い手技である開頭術の術後肺炎についてはほとんど知られていない。
目的
肺炎の発生率および肺炎と入院期間との関連の調査、リスク因子の特定、予測モデルとしてノモグラムの構築を目的とした。
方法
本研究集団は、American College of Surgeons のNational Surgical Quality Improvement Program の 2005 年から 2017 年のデータベースに基づいている。多変量ロジスティック回帰モデルおよび線形回帰モデルの両方を用いた。
結果
計 57,201 件の手術において術後肺炎の全発生率は 3.11%であった。リスク因子は、55 歳超、男性であること、体格指数(BMI)低値、糖尿病、機能的依存性、人工呼吸器依存、重度の慢性閉塞性肺疾患の病歴、高血圧症、全身性敗血症、白血球数 12,000 超、救急患者、American Society of Anesthesiologist 分類 3 度以上、全身麻酔、総手術時間 240 分超であった。10 の主要な因子をノモグラムに用いた(C 統計量= 0.803)。術後肺炎は、入院期間の延長と関連した。他の術後合併症と比較して、肺炎は入院期間の延長(4.7 日)に及ぼす影響が 2 番目に大きかった。
結論
本研究では、開頭術の術後肺炎に対する複数の術前リスク因子を特定した。男性であること、および BMI 低値を含む新たな因子により、さらなる研究が推奨される。この新規のノモグラムは、術後肺炎リスクを術前に評価するための信頼できるツールとしての機能を果たす。

開頭術の術後肺炎の発生率とリスク因子を調査し、その結果から肺炎リスクを術前に評価するためのノモグラムを作成した研究である。肺炎の発生率は3.11%、1,778例であった。ノモグラムは 10 のリスク因子をスコア化し、そこからリスクを計算するものである。実用後の評価を知りたいところである。
