カンジダ・パラプシローシス(Candida parapsilosis)による院内カンジダ血症:リスク因子、抗真菌薬感受性、および転帰

カンジダ・パラプシローシス(Candida parapsilosis)によるカンジダ血症患者の 30 日死亡と関連するリスク因子を明らかにするために、2000 年から 2010 年の後向き解析を実施した。C. parapsilosis による院内カンジダ血症患者 51 例を解析の対象とした。血液培養由来の全分離株が、ミカファンギンおよびフルコナゾール感性であった。全死亡率は 23.5%であり、極めて重度の合併症として、心内膜炎(5.9%)および眼内炎(5.9%)が認められた。多変量解析から、死亡と関連するリスク因子は APACHE II スコア >25(オッズ比[OR]43.9)および心血管補綴材留置(人工弁または人工血管)(OR 14.6)であった。心血管補綴材を留置している C. parapsilosis によるカンジダ血症の患者に対しては、速やかな外科的除去を検討すべきである。
監訳者コメント:
近年「non-albicans Candida」と呼ばれるカンジダ属が増加している。その中で C. parapsilosis は比較的感受性が良好とされ、一般的にフルコナゾールに感性を示し、ミカファンギンにも感性を示すが、時に治療不良例がみられる。本論文は C. parapsilosis による血流感染症患者を生存群・死亡群に分けて死亡のリスクを検討したもので、特に人工弁・人工血管が留置されていた場合に死亡のリスクが高かった。カンジダ属に限らず血流感染症においては常に人工物(ペースメーカーや人工関節など)の除去は念頭に置いておかなくてはならない。
