Mycobacterium xenopiのアウトブレイクは汚染水と関連している。M. xenopiは、配水管設備で形成されるバイオフィルムと相互作用があり、自由生活性Acanthamoeba属に寄生することが知られている。本研究では、M. xenopiとA. polyphagaアメーバの相互作用、さらにM. xenopiとヒト線維芽細胞であるHEL細胞の相互作用について調べた。光学顕微鏡を電子顕微鏡および共焦点顕微鏡と併用した検査によって、M. xenopiが本アメーバ種およびHEL細胞内に定着することが示された。M. xenopiとA. polyphagaのDNA比、およびM. xenopiとHEL細胞のDNA比をLight Cyclerで測定し、アメーバ内および細胞内でのM. xenopi増殖の倍加時間は、それぞれ5日および10日であることが示された。アメーバ内M. xenopiは、アメーバ原虫の被嚢後および出芽後も生存した。これらのデータは、M. xenopiがアメーバ内および細胞内に寄生する通性病原体(facultative pathogen)であることを示している。除菌を達成するためには、アメーバ内M. xenopi検査を行い、病院の配水管設備の汚染除去処理を適切に評価することが必要であると考えられる。

監訳者コメント:
アカントアメーバ内に寄生するM. xenopiまで除菌しないと再び増殖をはじめる可能性があるとの報告である。解決策としては、①宿主としてのアメーバの足場を無くす意味でバイオフィルムの分解と破壊、②アメーバ自身の除去、③M. xenopiの除去、④アメーバ内への移行が良好な抗菌薬(消毒剤)の使用が考えられる。本邦では水道の衛生基準にMycobacterium属についての規定がないために、本邦の汚染状況はあまり知られていない。
