予想を超えた臨床的に注意すべきクロストリジオイデス・ディフィシル（Clostridioides difficile）感染症（CDI）の増加（すなわちアウトブレイク）には、合意された定義が存在しない。病院の見地から CDI 発生率の増大がどの程度の規模になった場合にアウトブレイクとなるのかについての理解は、後向き解析にとって重要である。本研究では、文献においてアウトブレイクとして報告された CDI 発生率指標を明らかにし、Healthcare Cost and Utilization Project（HCUP）の退院データと比較した。文献では、アウトブレイク期間と基準期間との間で、発生率の中央値が 4.1 倍に変化すると報告していた（N ＝ 26、最低で 2 倍の発生率）。しかし、月間発生率におけるこの規模の変化は、HCUPのデータでは 1 回しか認められなかった（病院 20 施設、2012 年 から 2014 年）。したがって、文献では極端なアウトブレイクの例を示している可能性がある。

CDI のアウトブレイクを定義する研究。本研究からは CDI 発生率が 2 倍になることと 1SD の増加の両方でモニタリングするのがひとつの方法と考えられる。
