Multi-centre study on cultural dimensions and perceived attitudes of nurses towards influenza vaccination uptake

本研究では、2017 年に実施した調査（3,971 名）を用いて、3 か国のアジア人集団における看護師を対象に、文化的価値観が Health Belief Model の 4 つの概念（要素）およびインフルエンザワクチン接種にいかに影響するかを検討した。ワクチン接種率は、ブルネイ 33.5％、香港 35.6％、シンガポール 69.5％であった。Health Belief Model の 3 つの概念；1）自分がその状態になりやすいという信念（認知された脆弱性）、2）行動をとることが脆弱性や重大性を減らすという信念（認知された利益）、3）行動を促す要因への曝露（行動のきっかけ）は、ワクチン接種と正の関連を示した。Health Belief Model によって、集団主義とワクチン接種との間に、直接的な負の関連および間接的な正の関連がみられた一方で、力関係とワクチン接種との間では間接的な負の関連がみられた。文化的価値観、特に集団主義の点から、看護師のワクチン接種に関する研究にはHealth Belief Model の導入と応用が求められた。

Health Belief Modelは健康信念モデルとも呼ばれ、1950 年代に社会心理学者によって提唱されたもので、疾病を予防する活動や疾病のスクリーニング、疾病の管理を行うかどうか決定する際に、関連してくる 6 つの主要な概念（要素）を表したものである（詳しくは下記文献 1 を参照）。インフルエンザワクチンの接種を決定する時には他者からも影響を受けるが、看護師を対象とした本検討は、職場での集団的事情や力関係、そしてその背後にある文化的価値観も接種率に影響を及ぼしているのではないかとの立場に立っている。集団上の傾向や力関係は国によって異なっており、決定に関して個人が集団に依存する度合いも様々であるが、このような集団と個人との関係に着目した行動科学からワクチン接種をみることは興味深い。
