Impact of colonization pressure on acquisition of extended-spectrum β-lactamase-producing Enterobacterales and meticillin-resistant Staphylococcus aureus in two intensive care units: a 19-year retrospective surveillance

背景
保菌圧は集中治療室（ICU）獲得型多剤耐性菌（MDRO）のリスク因子の 1 つである。

目的
ICU 獲得型基質特異性拡張型β- ラクタマーゼ産生腸内細菌目細菌（extended-spectrum β-lactamase-producing Enterobacterales；ESBL-PE）およびメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）に対する保菌圧の長期的な個別の影響を評価すること。

方法
この後向き観察研究には、1997 年 1 月から 2015 年 12 月の間に ICU（内科および外科）2 室に入院したすべての患者が含まれた。入院時とその後毎週、直腸および鼻腔の監視培養を採取した。保菌または感染患者に対し、接触予防策が適用された。保菌圧は、全患者日に対する各 MDRO の陽性患者の患者日の比率として定義した。シングルレベルの負の 2 項回帰モデルを用いて、毎週の MDRO 獲得の発生率を評価した。

結果
対象患者 23,423 例のうち、ESBL-PE の保菌患者は 2,327 例（10.0％）で 660 例（2.8％）の獲得を含み、MRSA の保菌患者は 1,422 例（6.1％）で 351例（1.5％）の獲得を含んでいた。ESBL-PE の獲得は、1997 年の 0.51/1,000 患者曝露日から 2015 年の 6.06/1,000 患者曝露日へ増加していた（P ＜ 0.001）。対照的に、MRSA の獲得は 3.75/1,000 患者曝露日から 0.08/1,000 患者曝露日へと着実に減少していた（P ＜ 0.001）。期間レベルの共変数を調整すると、前の週の保菌圧は ESBL-PE に関しては MDRO の獲得に関連していたが（P ＜ 0.001［内科 ICU］、P ＝ 0.04［外科 ICU］）、MRSA に関しては関連していなかった（P ＝0.34［内科 ICU］、P ＝ 0.37［外科 ICU］）。ESBL-PE に関して、保菌圧の上昇は 100/1,000 患者日を超えると有意であった。

結論
保菌圧は ESBL-PE 獲得の発生率の上昇に関与していたが、MRSA には関与していなかった。本研究は、感染制御策は MDRO に合わせて特化すべきであることを示唆している。
