クロストリオイデス・ディフィシル（Clostridium difficile）感染症（CDI）は、入院患者にとって依然として脅威である。下痢（Bristol Stool Form Chart スコア 5 ～ 7 と定義）を有し、別の原因が考えられないすべての患者に対して、CDI の検査を行うこととした。サンプルは、検体容器の4 分の 1 を満たすこととした。検査室で CDI の検査を行うべきサンプルの決定における Bristol Stool Form Chart による下痢の定義の使用について評価を行った。本研究では、グルタミン酸脱水素酵素陽性で、毒素陽性のサンプルの 3 分の 2 が、Bristol Stool Form Chart スコア 5 未満と判定された。したがって、Bristol Stool Form Chart は、検査室において CDI の検査を行うべきサンプルの決定に用いるべきではない。

監訳者コメント：
CDI の検査を行う基準として Bristol Stool Form Chart による便の性状評価が行われ、スコア 5 以上での検査が推奨されており、日本の CDI 診療ガイドラインでも採用されている。レターでも指摘されているように、本研究では Bristol Stool Form Chart のスコア 5 未満の検体はトキシン陽性 C. difficile 保菌が含まれている可能性があり、そうであれば過剰診療につながる可能性もある。本研究をもって CDI 検査を Bristol Stool Scale での便の性状評価に用いるべきではないと結論付けることはできない。
