Hospital-wide outbreak of Burkholderia contaminans caused by prefabricated moist washcloths

2 つのキャンパスを有するドイツの大学病院で、バークホルデリア・コンタミナンス(Burkholderia contaminans)(バークホルデリア・セパシア[Burkholderia cepacia]Group K)の病院アウトブレイクが発生した。症例の定義は、2008 年 6 月 30 日から 10 月 21 日までに検査室に送付された何らかの臨床検体から、B. cepacia complex(BCC)が微生物学的に検出されることとした。recA 遺伝子シークエンス法によりBCCの菌種を同定し、パルスフィールド・ゲル電気泳動(PFGE)(SpeI による消化)でクローンを同定した。両キャンパスで合計 61 例が BCC 陽性症例と診断された。少なくとも 9 例の患者が、BCC による人工呼吸器関連肺炎を発症した。1 例にペーシング電極挿入部位の感染が認められ、4 例は敗血症を発症した。16 例が入院中に死亡したが、いずれもアウトブレイク菌株が原因ではないと考えられた。最終的に、BCC は集中治療室の患者に使用される既製のウェットタオルのパッケージ内から検出された。ドイツの保健医療当局はこの報告を受け、他の病院での感染を予防するため、EU の緊急警告システムである RAPEX を介して警告を発した。PFGE により、臨床検体および両キャンパスのウェットタオルに由来する分離株は同一クローンであることが判明した。汚染ウェットタオルの使用中止後は、新規症例の発生はみられなかった。比較的新しく導入された製品による今回の事例から、現行の規則は利用者保護に適切であるのかという問題が提起される。重篤な患者の場合は、医療用品の評価を入念に行うべきである。汚染製品に由来する感染がみられた際は保健医療当局への迅速な報告が必要であり、当該製品が欧州各国で流通しているような場合は RAPEX を介する警告を考慮すべきである。

タオルは実に多くの問題となる細菌を運ぶ。衛生管理の徹底が重要であるが、我が国でも患者もちのタオルの衛生管理まではしていない。
