閉鎖環境および半閉鎖環境でのノロウイルスのアウトブレイクにおける環境サンプル中のノロウイルス検出★

背景
環境表面は閉鎖環境および半閉鎖環境におけるノロウイルスのアウトブレイクの潜在的な感染媒体の 1 つである。環境サンプルの検査はアウトブレイクの制御に役立つ可能性がある。
目的
2017 年 1 月から 2019 年 3 月の間の、バルセロナ地方の閉鎖環境または半閉鎖環境(ナーシングホーム、学校、幼稚園、青少年の宿泊施設、病院および福祉医療センター)での急性胃腸炎のアウトブレイクにおけるノロウイルスによる環境汚染の程度を評価すること。
方法
前向きサーベイランス研究を行った。ノロウイルス急性胃腸炎のアウトブレイク 50 件のうち 46 件において、アウトブレイクの届け出がされた時と 10 日後に、閉鎖環境および半閉鎖環境の共用部分、トイレおよび台所の環境表面から環境サンプル(529 件)が採取された。環境サンプル採取に関する説明書を公衆衛生検査官に配布した。ノロウイルスは逆転写 PCR によって検出した。
結果
31 件(67.4%)のアウトブレイクにおいて、環境サンプルはノロウイルス陽性であった。ノロウイルスは、エレベーターのボタン(17 件中 4 件、24%)、トイレのハンドル(66 件中 16 件、24%)、手すり(34 件中 7 件、21%)で最も高頻度に検出された。初回のサンプル採取では陽性サンプルは主にトイレで確認され、2 回目のサンプル採取ではエレベーターのボタンとテレビのリモコン上でウイルスの残留がより多く確認された。ナーシングホームは、環境表面の汚染が最も多い(初回サンプルの 82%、2 回目のサンプルの 55%)タイプの環境であった。
結論
ウイルス検出の確率は、アウトブレイクの届け出または症状発現とサンプル採取との間の期間と無関係である。我々の結果は、閉鎖環境および半閉鎖環境での清掃手順と消毒における不備の可能性を示唆している。

ノロウイルスのアウトブレイクが発生した施設や学校での環境表面の汚染を調べた論文である。高頻度接触表面と言われる場所でウイルスが高頻度に検出されていたことは、改めて、手指衛生の重要性、環境の清掃・消毒の重要性を認識させてくれた。
