Implementing a multi-faceted framework for proprietorship of hand hygiene compliance in a network of South African hospitals: leveraging the Ubuntu philosophy

背景
低中所得国において、全病院レベルでの当事者意識の欠如および看護師不足を考えると、手指衛生の行動変容を大規模に実行するための理想モデルは不明である。

目的
この多面的な対策の目的は、手指衛生遵守に関して病院の説明責任を果たすことである。

方法
南アフリカの病院 50 施設において準実験研究を実施し（2015 年 11月から 2017 年 7 月）、逐次的な5 段階、すなわち、実行のガバナンスと集団の行動変容、グループ全体の系統的な状況分析、電子的支援による直接観察データの収集・解析アプリケーションの開発、着手と実行、説明責任を果たすガバナンスを組み入れた。手指衛生の 6 つの機会における病院内および病院間の相違を数値化し、記録された手指衛生遵守および行動のフィードバックを現場のチームに提供するために、病院の指導部に遵守データのダッシュボードを電子メールで週 1 回配信した。ベースライン（2016 年 7 月）と実施後（2017 年 7 月）の遵守率を比較した。

結果
病院の 16％（50 施設中 8 施設）で、ベースラインの手指衛生遵守率が 60％以下であったのに対し、実施後には病院の 48％（50 施設中 24 施設）で有意な改善が認められた（P ＜ 0.01）。13 か月の観察期間を通して、523,422 回の観察が記録され、平均観察率は 1,000 患者・日 あたり 277 ± 223であった。病院グループ全体の平均遵守率は、2016 年 7 月には 77.4％ ± 12.8％であったのが、2017 年 7 月には 85.2％ ± 8.8％となり、7.8％改善した（P ＜ 0.01）。

結論
南アフリカの多様な大規模病院グループにおける多面的手指衛生モデルの実施は、組織の体系および説明責任の変容、標準化された手指衛生遵守の管理、手指衛生の責任に導くフィードバックにつながった。

監訳者コメント：

手指衛生の施設内における定着には、労力と忍耐が必要である。各医療機関における手指衛生行動が文化として普遍的に定着するまでには年単位の時間を要する。大規模病院グループ間でこうした取り組みをベンチマーキングすると、競争意識が生まれ短期間でのレベルアップに繋がる。一方で問題はそこから高い水準を維持することである。
