日本の 3 次ケア病院の看護師における個人防護具使用の遵守率:ばらつきが生じる理由★

Adherence to personal protective equipment use among nurses in Japanese tertiary care hospitals: what determines variability?

背景

看護師は患者と頻繁に接触するが、看護師の個人防護具(PPE)遵守率は低く、このことが感染制御における大きな課題となっている。

 

目的

個々の特定の因子について PPE 遵守率に及ぼす相対的影響を検討すること。

 

方法

逐次的 2 段階混合法デザインを用いた。定性的研究として、2018 年 5 月 から7 月にかけて半構造化面接を実施した。定量的研究として、2019 年 1 月 から3 月にかけて、全国横断調査を実施し、日本の 3 次ケア病院 28 施設の看護師 735 名に質問票を郵送した。

 

結果

定量的研究において、解析可能な 435 件(59.2%)の回答が得られた。線形回帰分析により、知識の欠如により「患者に感染症の徴候がみられる場合、標準予防策が基本的な感染症対策であり、これらの対策は感染症が認められなければ終了できる」と考えていることは、低い PPE 遵守率と有意に関連していた一方、抗菌薬耐性菌アウトブレイク、またはアウトブレイクによるによる病棟閉鎖、ならびに「私が感染症の拡散の原因に決してなってはならない」という信念は、高い PPE 遵守率と有意に関連していた。各項の標準化偏回帰係数のβおよび t 値はそれぞれ、-0.344、-7.784、0.090、2.089、0.088、2.018 であった。

 

結論

本調査では、PPE 遵守率に寄与する看護師に関連する因子を系統的に同定した。良好なアウトカムを得るための実用的なアプローチとして、適切な PPE 使用についての十分な知識の提供、ならびにアウトブレイクや病棟閉鎖の経験の共有により、看護師の教育を行うことを我々は提案する。

監訳者コメント:

防護具着用の遵守率の低さの理由として、これまでの報告では、着用時の不快感、直ぐに使えない、業務量や時間の余分な増加、感染対策への知識不足が関与しているがわかっている。さらに、eラーニングのみでは防護具着用の遵守率を上げないことも報告されている。本論文は日本における防護具遵守率に関する全国的な調査であり、日本における急性期病院での防護具着用の遵守率の低さの要因に関する調査研究である。感染対策への知識と同時に、過去のアウトブレイクの経験や感染予防への意識は大きな遵守率向上への要素であり、遵守率改善には教育や訓練、意識改革、行動変容のための方策の検討など、まだまだ多くの課題が残されている。
