Impact of an empiric therapy guide on antibiotic prescribing in the emergency department

今回の準実験研究では、救急部における経験的治療の指針の実施前後の各 3 か月間、適切な抗菌薬処方について比較した。米国感染症学会（Infectious Diseases Society of America；IDSA）ガイドラインに基づく全体的な抗菌薬処方の適切性は、経験的治療の指針の実施後には20.5％有意に増加した（P ＜ 0.001）。市中感染肺炎および蜂窩織炎の処方は、それぞれ33.1％（P ＜ 0.001）、35.5％（P ＝ 0.002）改善した。広域スペクトル抗菌薬の使用率は13.6％減少した（P ＜ 0.001）。介入後、処方者の 90.5％が、IDSA ガイドラインに基づく適切な処方を少なくとも 75％達成した（P ＜ 0.001）。経験的治療の指針の実施後、適切な抗菌薬処方および広域スペクトル抗菌薬の使用率は有意に改善した。
