冠動脈バイパス術前にチカグレロルまたはクロピドグレルの投与を受けた患者における感染性合併症

抗血小板薬のチカグレロルは殺菌活性を有することが最近確認されてきており、in vitro と in vivo のマウスモデルで実証され、さらなる臨床試験が必要とされている。本研究の目的は、チカグレロルまたはクロピドグレルを術前に投与された患者において、冠動脈バイパス術後の感染性合併症を評価することであった。多施設共同試験において、単独の冠動脈バイパス術より前にチカグレロルまたはクロピドグレルを投与されたすべての成人患者を対象として選定した。傾向スコアマッチングを使用した。評価項目は、すべての胸骨創感染症、深部胸骨創感染症、および術後に院内で使用された全抗菌薬であった。含まれた 2,311 例の患者のうち、1,293 例(55.9%)がクロピドグレルの、1,018 例(44.1%)がチカグレロルの術前投与を受けた。全体の解析および傾向スコアマッチング解析の両方において、チカグレロル群での感染性合併症の発生率は、クロピドグレル群と同程度であった。我々の知見は、冠動脈バイパス術を受けた患者において、チカグレロルの臨床的に意義のある殺菌効果を裏付けるものではなかった。

監訳者コメント:
先行研究によると、チカグレロルはブドウ球菌を含む主にグラム陽性球菌に殺菌能を有するとされている。ただし試験管上の最小発育阻止濃度に比べ、通常投与量で達成される血中濃度は低いことが判明していた。本研究のアウトカムは胸骨創感染であり、微生物の種類は問わない。また術前 2 日以内にチカグレロル、クロピドグレルを投与していた患者はさらに限定される。対象患者、アウトカムの対象としての微生物と感染症の種類、併用していた抗菌薬の種類などを変更した今後の解析が待たれる。
