背景
医療関連感染のサーベイランスは手術部位感染(SSI)の低減と関連している。
目的
Korean Nosocomial Infection Surveillance System(KONIS)が SSI に及ぼす影響を評価するために、その導入以降の KONIS を検討すること。
方法
2008 年から 2012 年の胃切除術、人工股関節全置換術、人工膝関節全置換術施行後の SSI データを分析した。プールした SSI 発生率を年別に計算した。また、連続 3 年以上 KONIS に参加している病院についても同じ分析を行った。SSI のリスク因子を補正することにより標準化した年別の SSI 発生率を計算した。SSI の傾向について Cochrane‐Armitage 検定を用いて分析した。
結果
胃切除術後の SSI 発生率は 3.12%(16,918件中 522 件)であった。5 年間の SSI 粗発生率の有意な低下傾向が認められた。この傾向は 3 年以上参加した病院の分析でも明らかであった。人工股関節全置換術後の SSI 発生率は 2.05%(7,656 件中 157 件)であり、2008 年から 2012 年に有意に低下した。人工股関節全置換術後 SSI のリスク因子は、全米病院感染サーベイランスのリスク指標、外傷、再手術、および年齢(60 ~ 69歳)であった。人工膝関節全置換術後の SSI 発生率は 1.90%(7,648 件中 152 件)であり、この場合も 5 年間に有意に低下した。しかし、リスクを補正した SSI 分析ではいずれの外科手術についても有意な低下は認められなかった。
結論
KONIS による積極的なサーベイランスの結果、胃切除術後および人工関節置換術後の SSI発生率は 5 年間で低下した。

各国のサーベイランスの情報を知るのは興味深いが、医療提供のありかたなどデータをみる上で必要な療養環境の内容やサーベイランスの方法が標準的なものかどうかを認知しておくことが重要である。
