Impact of a checklist used by pharmacists on hospital antimicrobial use: a patient-level interrupted time series study

背景
薬剤耐性につながる抗菌薬の誤用は、臨床医の関心を高めている。新たなツールの開発による抗菌薬適正使用支援プログラムの改善が、この問題解決の一部となる可能性がある。
目的
病棟の臨床薬剤師用の抗菌薬チェックリスト導入後の、入院患者における抗菌薬使用について評価すること。
方法
抗菌薬治療に関する病院薬剤師の評価を標準化するために、最適な抗菌薬使用の質的指標に基づくチェックリストを導入した。対照期間と介入期間に入院した成人の抗菌薬使用の数的指標を、個々の患者データの分割時系列分析により評価した。主要評価項目は、対象患者における在院 1,000 日あたりのあらゆる種類の抗菌薬治療日数（DOT）とした。副次評価項目は、基質特異性拡張型の抗菌薬治療日数（DOT-ES）、あらゆる種類の抗菌薬治療期間（LOT）、および薬剤師による介入回数とした。
結果
患者 1,619 例を対象とし、内訳はチェックリストの導入前の期間が 800 例、導入後の期間が 819 例であった。点推定値とその 95％信頼区間（CI）により示されるように、DOT、DOT-ES、LOT の傾向性に変化はなかった。DOT の数値に変化はみられなかったが、DOT-ESは 118 日（95％CI 209 ～ 28）減少、LOT は 51 日（95％CI 97 ～ 4）減少した。さらに、抗菌薬に関する薬剤師の介入が 18.7％（95％CI 14.0 ～ 23.5）増加し、経過記録の記載が 32.3％（95％CI 27.8 ～ 36.8）増加した。
結論
病棟の臨床薬剤師により使用される抗菌薬チェックリストの導入によって、あらゆる種類の DOT は減少しなかったが、DOT-ES および LOT は減少した。
