バンコマイシン耐性腸球菌(VRE)を保菌している患者に対する接触予防策の最適な期間は不明であり、個々の患者の特性のみが保菌延長のリスクを予測するのではない可能性がある。ICU 入室時に VRE の検査を受けた成人患者のコホートを用いて、ユニットレベルのバンコマイシンの使用と VRE の持続的な保菌との関連性を検討した。ユニットレベルでのバンコマイシンの使用がより多いと、患者個々のバンコマイシンの使用量およびユニットの VRE の密度と関係なく、VRE の保菌は有意に延長した(P = 0.03)。

監訳者コメント
VRE 保菌者本人のバンコマイシンの使用や、周囲の患者を含めたユニット(局所)の VREの検出度合いよりも、ユニット全体のバンコマイシンの使用量が、VRE の保菌期間を長くしたという報告である。この結果を敷衍すれば、患者個人の抗菌薬の使用のみならず、周囲の患者の抗菌薬の使用も、多剤耐性菌(MDRO)の長期保菌を予想させることになる。患者・周囲患者の腸内細菌叢、環境中の細菌叢の情報はないが、これらの密接な関連も推測できる。加えて、院内の抗菌薬適正使用活動は、特定の種類の抗菌薬を使用している患者に特に注意が集まりがちであるが、それ以外の薬剤が高密度・高頻度に使用されている状況も、十分コントロールされなければならないことを示しているであろう。
