Effects of the application of vitamin E and silicone dressings vs conventional dressings on incisional surgical site infection in elective laparoscopic colorectal surgery: a prospective randomized clinical trial

目的
結腸直腸の待機的腹腔鏡下手術を受ける患者において、従来の創被覆材とビタミン E・シリコン創被覆材が切開手術部位感染症（SSI）に及ぼす効果を比較すること。
患者および方法
前向き無作為化試験を実施した。患者をビタミン E・シリコン創被覆材群と従来の創被覆材群の 2 群に無作為に割り付けた。切開部 SSI、術後疼痛、急性期反応物質について調べた。
結果
患者計 120 例を本研究に組み入れた（各群 60 例）。切開部 SSI 発生率は、ビタミン E・シリコン創被覆材群では 3.4％、従来の創被覆材群では 17.2％であった（P ＝ 0.013）。ビタミン E・シリコン創被覆材群における感染創の培養ではバクテロイデス フラジリス（Bacteroides fragilis）のみの増殖がみられたのに対し、従来の創被覆材群の感染創の培養では複数菌の増殖がみられた。術後 48 時間の術後疼痛の平均は、ビタミン E・シリコン創被覆材群では 27.1 mm（標準偏差［SD］10.7 mm）、従来の創被覆材群では 41.6 mm（SD 16.9 mm）であった（P ＜ 0.001）。術後合併症を呈した患者を除外しても、ビタミン E・シリコン創被覆材群の方が白血球数（WBC）およびC反応性タンパク質（CRP）は低値であった。
結論
結腸直腸の待機的腹腔鏡下手術後の Pfannestiel 切開創を被覆するのにビタミンE・シリコン創被覆材の使用は、切開部 SSI 発生率の低下、術後疼痛の軽減、CRP 値およびWBC 数の低下をもたらした。
