Bacterial contamination rate and associated factors during bone and tendon allograft procurement from Spanish donors: exploring the contamination patterns

背景
摘出中の同種移植片の汚染は、組織バンクの利用を制限する主要因子の 1 つである。汚染率は、考慮される病院、国または年度に関係なく、持続的に髙いままである。
目的
スペイン人ドナーから摘出された、骨および腱のサンプルの汚染に関連する因子を分析すること。
方法
2014 年から 2017 年の間の、ドナー 102 例から摘出された骨および腱のサンプル 1,162 例のデータを、病院データベースから後向きに収集した。同じドナーの異なる解剖学的部位から摘出されたサンプル間の汚染の記述統計、潜在的な関連因子および相関を分析した。
結果
全体で、227 例（19.54％）の摘出サンプル（骨サンプル 131 例［18.49％］、腱サンプル 96 例［20.92％］）が培養陽性であり、廃棄された。男性（オッズ比［OR］2.023、P ＝ 0.019）、ドナーあたりの摘出サンプル数が 10 を超えること（OR 1.997、P ＜ 0.001)、240 分を超える摘出時間（OR 1.755、P ＝ 0.001）は、より高い汚染率と独立して関連する因子であった。一方、組織サンプルの種類が「骨付き膝蓋腱」であることは、有意に低い汚染率に関連していた（OR 0.446、P ＝ 0.001）。汚染されたサンプルの一定の局在性と細菌種の一致にも、有意な相関が認められた。
結論
総摘出時間、摘出順序、摘出されたサンプル数、ならびに摘出サンプルの解剖学的部位といった摘出手順に関連する因子は、同種移植片汚染において主要な役割を果たしている。本研究で分析された汚染パターンを指針として、手順をさらに最適化することは、組織バンクの利用を高めるのに役立つはずである。
