Hospital-acquired Clostridium difficile infection: an institutional costing analysis

背景
医療関連クロストリジウム・ディフィシル（Clostridium difficile）感染症（HA-CDI）は、一般的な感染症であり、医療制度に経済的負担を課している。
目的
費用は全入院期間によるものとする時間固定型の統計モデルと、入院、HA-CDI 診断、退院までの期間を調整し、HA-CDI の費用は診断後の期間によるものとする時間依存型の統計モデルを比較することにより、HA-CDI による病院の財務費用を推定することである。
方法
Ottawa Hospital に 72 時間を超えて入院した患者（15 歳以上）の臨床データおよび入院原価計算データを用いて、後向きコホート研究を実施した（2008 年 4 月から 2011 年 3 月）。Kaplan-Meier 生存曲線を用いて、通常の最小二乗回帰モデルおよび一般化線形回帰モデルによる 2 つの時間固定分析を、2 つの時間依存分析と対比させた。
結果
入院は計 49,888 件で、366 例（0.73％）が HA-CDI を発症した。時間固定モデルによる推定総費用（カナダドル）は、1 例あたり 74,928 ドルであったのに対し、時間依存モデルでは 28,089 ドルで、この数値は HA-CDI を発症していない患者よりも 1.47 倍多かった（増分費用は 1 例あたり 8,997 ドル）。Ottawa Hospital における HA-CDI と関連する年間の総施設費は、時間固定モデルでは 1,007 万ドル、時間依存モデルでは 162 万ドルであった。
結論
HA-CDI と関連する費用を計算する場合、入院、診断、退院までの期間を調整すると、HA-CDI と関連する推定施設費は大幅に低減される。
