Effect of using fidaxomicin on recurrent Clostridium difficile infection

フィダキソマイシンはクロストリジウム・ディフィシル（Clostridium difficile）感染症（CDI）の治療に認可された大環状抗菌薬の 1 つである。英国では、フィダキソマイシンは重度の CDI または再発のために確保されていることが多い。バーミンガムのクイーン・エリザベス病院において、2017 年から 2018 年の間のフィダキソマイシンの全経過をレビューした。38 例の患者がフィダキソマイシンの投与を受け、軽度の CDI の初回エピソード中にフィダキソマイシンを投与された場合は 64％が治療に反応した。逆に、すべての再発性 CDI 患者はフィダキソマイシンによる治療に失敗した。重度の CDI へフィダキソマイシンを使用すると、42％の患者しか反応せず、結果に一貫性はなかった。これらの結果により、フィダキソマイシンは患者の初回エピソード中の軽度の CDI の治療として最も適していることが示唆される。

監訳者コメント：
わが国で 2018 年に発表された CDI 感染症診療ガイドラインでは、フィダキソマイシンは重症例、再発例、難治例などに推奨されている。ところが本研究ではフィダキソマイシンはこれらの CDI において十分な有効性が期待できないとしている。またアブストラクトには記載されていないが、本文中では糞便移植がコストパフォーマンスも高くて良いと主張している。わが国でも今後フィダキソマイシンの使用知見が集積され、その位置付けが徐々にはっきりしてくるだろう。
