手術器具のコンタミネーション除去のモニターのためのニンヒドリンの詳細な評価

背景
イングランド保健省の新しいガイダンスである Choice Framework for Local Policies and Procedures(CFPP 0101)においても、滅菌サービス部門で処理した手術器具の蛋白質除去の効果の確認にニンヒドリンが利用できるとされている。
目的
外科手技を介して変異型クロイツフェルト・ヤコブ病(vCJD)が伝播する可能性があることから、推奨されている蛋白質検出法を再評価する必要がある。
方法
本稿では、検査室および滅菌サービス部門でのニンヒドリンを用いた蛋白質検出の感度と適用可能性に関する研究について報告する。スワブ法による蛋白質除去効果についても評価する。
結果
ニンヒドリンによる蛋白質検出感度は低いことが示された。溶液中のウシ血清アルブミン(BSA)の検出限界は 205 μg/mL であり、アルギニンの検出限界は 6 μg/mL であった。1,000 μg未満の蛋白質をピペットで直接バイアルに注入した場合は、市販のニンヒドリンキットではラット脳ホモジネートと BSA のいずれも検出することができなかった。水で湿潤させたレーヨン製スワブを用いるスワブ法では、手術器具(N = 6)からの蛋白質(50 μg)除去効果は不十分であり、残存付着率は BSA 32 ± 4%、フィブリノゲン 61 ± 5%であった。0.5%洗剤(Triton X-100)溶液に浸漬したスワブでの除去効果はやや良好であり、残存率は BSA 20 ± 3%、フィブリノゲン 24 ± 2.8%であった。
結論
現在、滅菌サービス部門で使用されているニンヒドリンキットは、残存蛋白質の検出効果は不十分であるが、これは、ニンヒドリンによる蛋白質検出感度が低いことだけでなく、スワブ法による付着蛋白質の除去効果が低いことにもよる。全般的に、ニンヒドリンは検査室試薬としても、蛋白質検出キットとしても蛋白質検出感度が高くなく、コンタミネーション除去の評価に使用した場合は偽陰性件数が増加する。

監訳者コメント:
プリオンの完全な感染性の消失ができるのは焼却のみであり、完全な不活化は困難である。滅菌に先立っての洗浄の徹底は当然のことではあるが、極めて重要である。日本での対策の実践においては、「洗浄評価判定ガイドライン 2012 年 8 月」(日本医療機器学会、滅菌技師認定委員会)がその指針となると思われる。
