Nosocomial Mycobacterium tuberculosis transmission by brief casual contact identified using comparative genomics

本稿では、短時間の軽い接触による結核菌（Mycobacterium tuberculosis）の院内伝播による 1 症例を報告する。山形県においてルーチンに行われているvariable number tandem repeat検査により、患者 2 例から検出された結核菌の臨床分離株が互いに区別できない遺伝子型を示すことが分かった。両患者には、同じ日に同じ病院の待合室に居合わせたという疫学的な関係があった。比較ゲノミクスを行ったところ、2 つの分離株間の一塩基バリアントはわずか 2 個のみであったため、上記の待合室で最近、結核伝播が発生したと結論付けられた。これらの結果から、感染性のある結核患者の物理的隔離は、短時間の軽い接触による予測外の院内伝播を予防するためにも必須の制御策であることが示唆された。

結核菌の全ゲノム解析によって、病院の待合での短時間の接触が感染契機であることが明らかになったという貴重な報告である。接触の状況とともに、診断がつかない段階で投与されていたフルオロキノロンによって、キノロン耐性変異が生じていた結果も述べられている。是非本文を参照されたい。
