Application of the Theoretical Domains Framework to identify factors that influence hand hygiene compliance in long-term care

背景
医療従事者の手指衛生遵守は患者の安全のために重要であるが、遵守は不十分である。それにもかかわらず、長期ケア施設では手指衛生遵守について十分には研究されていない。
目的
行動変容のフレームワークである Theoretical Domains Framework を適用し、長期ケア施設における医療従事者の手指衛生遵守の修正可能な促進因子および阻害因子を特定すること。
方法
カナダ、オンタリオ州の長期ケア施設において、医療従事者の手指衛生遵守の促進因子および阻害因子について、医療従事者への質問票を用いて調査した。質問票の結果の説明は、複数の関連する心理学的行動変容理論の構成概念の統合に基づく Theoretical Domains Framework による。
結果
質問票により特定された阻害因子は、Theoretical Domains Framework の領域の環境文脈と資源（時間的制約、仕事量、環境規制）と整合した。質問票の結果から特定された促進因子は、Theoretical Domains Framework の領域の社会的／専門的役割とアイデンティティ（医療従事者に期待されること）、結果に対する信念（自身または他者への微生物伝播のリスク）と整合した。
結論
長期ケアにおける手指衛生遵守に対していくつかの阻害因子が存続している。Theoretical Domains Framework などの、行動変容の理論を説明するフレームワークは、それらの阻害因子の特定に有用となり得る。本研究によって、Theoretical Domains Framework と整合したいくつかの重要な行動の構成概念が特定された。これは、新たな手指衛生介入を策定する際に目標とすることができる。
