Implementation of a quality improvement project using the patient as the observer to improve hand hygiene compliance in ambulatory care practices

背景

衛生管理の訓練を受けた観察者は、診察室のドアが閉まると視線が遮られるため、外来環境における手指衛生遵守のモニタリングには依然として課題がある。今回の質改善プロジェクトは、外来環境の日常業務や業務の流れに沿う手指衛生遵守改善プログラムの実施に焦点を置いた。

方法

文献の調査後、看護指導者と感染予防チームが、32 の外来診療において「患者を観察者とする」手指衛生遵守プログラムを実施した。

結果

患者は 281,000 件の観察を終え、全遵守率は 90％以上であった。職務ごとの全遵守率の平均は、医療提供者では 91％、看護師では 89％、医療助手／技術者／その他では 91％であった。「あなたへのケア前」と「あなたへのケア後」の平均遵守率はそれぞれ、医療提供者では 92％、89％、看護師では 90％、87％、医療助手／技術者／その他では 92％、89％であった。

考察

本研究では、外来環境において、患者を観察者とする手指衛生遵守改善プログラムの実施が、うまく適用できることが示された。

結論

外来環境において、観察者として患者が関与することにより手指衛生遵守を効果的に監視することできる。

手指衛生の直接観察において、患者が観察者となるのは画期的な方法である。医療関連感染症の防止において患者参画は非常に重要な要素である。手指衛生の直接観察に限らず、日本でもこのような患者の感染対策への参画を評価する取り組みが行われると面白いだろう。
