Broad-spectrum antibiotic prescriptions are discontinued unevenly throughout the week

院内での広域スペクトルの抗菌薬（抗 MRSA 薬、カルバペネム系抗菌薬、ピペラシリン／タゾバクタム）の処方パターンを調べるため、大阪大学医学部附属病院で 1 週間を通じた抗菌薬開始および中止の分布に関するデータを収集した。開始数には平日の曜日間で有意な差は認められなかった。しかし、広域スペクトルの抗菌薬の中止は火曜日または休日後 2 日目に偏っていた。本研究は、広域スペクトルの抗菌薬は週末や休日をまたいで継続され、その後中止される傾向があることを示唆している。これは、おそらく医療上の適応以上に行動要因によるものであり、今後の抗菌薬適正使用のイニシアチブにおいて取り組む必要があるといえる。

抗菌薬の中止時期を決めるにあたり、週末や休日が影響することは、多くの医療従事者が経験、あるいは首肯することではないだろうか。本研究はそれを明らかに「見える」ものとして提示することができたといえよう。
