Efficacy of antibiotic prophylaxis against ventilator-associated pneumonia

集中治療室において、人工呼吸器関連肺炎（VAP）は最も重要な問題の 1 つである。急性期脳卒中を有する神経系障害患者 84 例（グラスゴー昏睡スコア 8 点以下）を二重盲検臨床試験に組み入れた。介入群の患者には、ピペラシリン／タゾバクタム（4 g ／ 0.5 g）を挿管時および 12 時間後に投与した。早期 VAP（挿管後 4 日以内）の発生率は、介入群では機械的換気 1,000 日あたり 9.2 エピソード、対照群では機械的換気 1,000 日あたり 26.9 エピソードであった（オッズ比［OR］0.217、95％信頼区間［CI］0.056 ～ 0.085、P ＝ 0.028）。ピペラシリン／タゾバクタムの予防投与は、早期 VAP を減少させうるが、その有益性は晩期 VAP にまで及ばない。

監訳者コメント：
急性期脳卒中の患者を対象とした二重盲検臨床試験が行われた。患者背景にばらつきが大であるなかで、有意差を持った抗菌薬予防投与が VAP の減少に寄与したという結果は ICU 滞在日数の減少など、医療費の抑制にも寄与すると思われた。
