A systematic review of surgical site infections following day surgery: a frequentist and a Bayesian meta-analysis of prevalence

背景
1990 年以降、いくつかの研究により、日帰り手術に関連する安全性および患者の満足度に焦点が当てられた。しかし、今日まで、手術部位感染症(SSI)の全有病率を検討したメタアナリシスは行われていない。
目的
日帰り手術後の SSI の全有病率を、手術手技の種類にかかわらず推定すること。
方法
日帰り手術後の SSI の有病率について、2016 年 6 月以前のすべての研究を対象として、手術手技の種類にかかわらず、システマティックレビューおよびメタアナリシスを実施した。DerSimonian と Laird の方法を用いた統合ランダム効果モデルにより、全有病率を推定した。二重逆正弦変換を用いて、割合の分散の安定化を行った。感度分析を実施してこの方法の頑健性を検証した後、単変量および多変量メタ回帰を用いて、発表日、研究実施国、研究対象集団、専門科の種類、コンタミネーションのレベル、日帰り手術後の患者の術後来院日、および病院ケアの実施時間の影響を検討した。
結果
観察研究および無作為化研究の両方を含め、90 報の論文を分析した。日帰り手術を受けた患者の SSI 全有病率は推定で 1.36%(95%信頼区間 1.1 ~ 1.6)であり、ベイズ確率は 96.5%が含まれ、1 ~ 2%の区間であった。論文の発表日は SSI 有病率と関連しており(相関係数 -0.001、P = 0.04)、また専門科(消化器科手術と消化器科以外の手術との比較)には SSI 有病率と関連する傾向が認められた(相関係数 0.03、P = 0.064)。
結論
本メタアナリシスから、手術手技にかかわらず日帰り手術後の SSI 有病率は低いことが示された。

監訳者コメント
日帰り手術は 1960 年代に米国や英国等で開始され、その手技は確実に発展してきている。日帰り手術のメリットは、患者医師共に満足度が高く、予定の立てやすさ、低コストなことである。しかしながら、そのメリット以外に、術後の合併症、特に手術部位感染は退院後に発生するため注意深い経過観察が必要である。日帰り手術の開始後、年代が進むにつれて SSI の発生率は減少し、通常の入院手術とほぼ変わらないか、低いことが報告されている。しかしながら、より健康な患者が日帰り手術を選択されているというバイアスの結果、SSI が低いとする報告もある。本論文は日帰り手術の SSI に関する初めてのメタ解析の論文であり、その意義は大きい。今後入院手術と日帰り手術の両者について同一の手術手技での比較研究が必要である。
