Factors that impact on the burden of Escherichia coli bacteraemia: multivariable regression analysis of 2011-2015 data from West London

背景
抗菌薬耐性と院内感染が疾患負荷に及ぼす影響が懸念される中で、英国において大腸菌（Escherichia coli）菌血症の罹患率は高まりつつある。
目的
西ロンドンにおける大腸菌菌血症の負荷と重症度に対する、院内発症型大腸菌血流感染症および大腸菌の特異的な抗菌薬耐性パターンの相対的寄与を明らかにすること。
方法
2011 年から 2015 年の大腸菌菌血症の全症例において、患者および抗菌薬感受性に関するデータを収集した。感染症のカテゴリー（院内発症型または市中発症型）と、入院期間、集中治療室入室、30 日全死因死亡率の関連を検討するために、多変量ロジスティック回帰分析を用いた。
結果
研究期間に大腸菌菌血症の罹患率は 76％増加し、これは主に市中発症型症例による。キノロン系、第 3 世代セファロスポリン系、アミノグリコシド系に対する耐性も研究期間を通して増加し、市中発症型と院内発症型の両方で発現した。院内発症型、キノロン系または第 3 世代セファロスポリン系に対する非感受性は、高齢であることと同様に入院期間延長の有意なリスク因子であった。7 日死亡率は 7％、30 日死亡率は 12％であった。高齢、併存疾患スコア高値、3 つのクラスの抗菌薬に対する耐性株による菌血症はすべて、30 日死亡率の有意なリスク因子であった。
結論
多剤耐性、加齢、併存疾患は、有害転帰の主要な誘因であった。大腸菌菌血症の増加は主に市中発症型感染症によってもたらされており、市中発症型症例の予防イニシアチブは、大腸菌感染症の量的負荷を減らすことに重点を置くべきであろう。

市中で伝播する大腸菌などの耐性菌には、健常人でも感染するリスクがあり広範囲のヘルスケア領域でその蔓延が問題となる。在宅医療や介護医療の中での過剰な抗菌薬療法を控えることで耐性化を抑止する必要がある。
