Carbapenemase-producing Enterobacteriaceae in Australian hospitals: outcome of point-prevalence screening in high-risk wards

オーストラリアの病院におけるカルバペネマーゼ産生腸内細菌科細菌（CPE）感染症の報告が増えているが、有病率は不明である。2016 年に Victorian 病院は高リスク病棟（集中治療室、血液病棟、移植病棟）でCPE 点有病率調査を実施した。43 の病院が 134 件の調査を行い、高リスク患者 2,342 例のうち 1,839 例（79％）にスクリーニングを実施した。他の病棟で 24 件の調査も実施した。患者の同意を得ることができなかったことが、不参加の主な理由であった。高リスク病棟において CPE 症例は検出されなかったが、他の病棟で 3 例が確認された。高リスク病棟では有病率が低いため、継続的なスクリーニングは推奨されない。患者同意の過程のレビューにより、選択的スクリーニングが推進される可能性がある。

本結果をふまえ、Victorian 病院は定期的な CPE 点有病率調査（PPS）をやめ、保菌者が検出された場合は、最終の探知から 4 週間が過ぎるまで継続して毎週調査を行うことをガイドラインに盛り込んだ。
