高流量式鼻カニューレの使用が増えつつあるが、気体の流量が大きいため、感染性粒子のエアロゾル化および感染症の拡散について懸念が生じている。本無作為化対照クロスオーバー非劣性試験(N = 20)では、グラム陰性菌による肺炎の重症患者に対する高流量式鼻カニューレと従来の酸素フェイスマスクの使用について、生きた細菌による環境汚染の程度を比較評価した。本研究の結果から、高流量鼻カニューレの使用により、グラム陰性菌または総細菌による空気中または接触表面の汚染(落下細菌)に増加は認められないことが示され、このことから、さらなる感染制御策は必要ないことが示唆される。

監訳者コメント:
高流量式鼻カニューレは、10 ~ 60 L/分の高流量を流すことで、呼吸不全患者への酸素療法としてその使用頻度が増加している。酸素マスクが6 ~ 8 L/分でることと比較すると、高流量酸素により呼吸器分泌物のエアロゾル化がおこり、空気感染の危険性が考えられる。本論文では、グラム陰性菌による肺炎患者においてどちらの酸素療法においても飛散する細菌数は変わらず、高流量式鼻カニューレ使用時の感染対策は従来と変わらないとしている。しかしながら、高流量酸素投与での SARS ウイルスやインフルエンザウイルスによる空気感染は報告されており、この論文からの結論には限界がある。
