Utility of MALDI-TOF mass spectrometry in an outbreak investigation of acute endophthalmitis following intravitreal injection

ベバシズマブは腫瘍血管新生を阻害する組換えヒト化モノクローナル抗体であるが、脈絡膜新生血管の治療に承認外使用が可能である。この薬物の使用後にアウトブレイクが報告されているが、原因は製造時のコンタミネーションか、単一のバイアルから異なる患者に複数回の投与を行なったためのいずれかであった。当施設において、著者らはベバシズマブの硝子体内注射後に発生した 1 件の眼内炎アウトブレイクを調査し、マトリックス支援レーザー脱離イオン化飛行時間型質量分析法（MALDI-TOF MS）を用いて、ステノトロホモナス・マルトフィリア（Stenotrophomonas maltophilia）が原因菌であることを迅速に同定し、ベバシズマブの汚染されたバイアルが感染源であることを確認した。本研究の結果は、感染源および病原体の迅速な同定が、適切な感染対策に必要であることを強調するものであった。

監訳者コメント：
眼内炎患者から採取された少量の硝子体内容物から、原因微生物を培養で証明できる例は少なく、技術的にも容易ではない。今回の事例では、一部の例でグラム染色が陽性となり、硝子体液を増菌するステップを経て、検体から直接 MALDI-TOF MS を行い、28 例全例で高いスコアでそれを証明しえた。最終的に本菌が発育したのは 10 例であったので、MALDI-TOF MS の有用性は高いものであった。そしてこのような MALDI-TOF MS の応用を迅速に行い、感染対策に応用できたシステムならびに意識を評価したい。
