Scrubbing technique for needleless connectors to minimize contamination risk

本研究の目的は、汚染リスクを最小化するためのニードルレスコネクターの適切なスクラブ手技を検討することであった。細菌の物理的除去を目的とする極めて効果的なスクラブ手技を実証するため、ニードルレスコネクターをゲオバチルス・ステアロサーモフィルス（Geobacillus stearothermophilus）の芽胞で汚染し、その後スクラブした。本研究により、最も細菌の除去率が高いのは、動脈圧迫止血とほぼ等しい力をアクセスポートに加え、アルコール綿でアクセスポートを一直線にスクラブし、さらに新しいアルコール綿を使ってこの手順をもう一度繰り返す方法であることが示された。

監訳者コメント：
needleless connector、いわゆる閉鎖式輸液セット用のコネクター部分は、表面に凹凸があるため菌を十分に拭うことができず、さらに複雑な内部構造のためそれが繁殖しやすく、カテーテル関連感染症・菌血症の原因となることがかねてより指摘されてきた。本検討では、芽胞を付着させたコネクターをモデルとし、様々なスクラブ法による除去率の違いを調査している。臨床上、貴重な情報であると思われる。一読をお勧めする。
