病院に再入院したメチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)の既知の保菌者を対象に、MRSA の保菌持続性に関して期間およびリスク因子を検討するため、後向きコホート研究を実施した。MRSA の保菌は、既知の保菌者の大部分において最後の退院日から 2 年まで持続したため、少なくともこの期間は再入院時のスクリーニングを実施すべきである。長期入院歴のある高齢患者は持続性保菌のリスクが高いため、重点を置いて標的スクリーニングの取り組みを実施すべきである。既知の保菌者における MRSA の持続性保菌リスクを減らすうえで、適切な時期の退院計画が重要となる。

監訳者コメント:
MRSA 保菌の期間は、再入院時の感染拡大リスクを考える際に極めて重要である。過去の文献においても 1 年後で 50%前後、2 年後は 20 〜 30%前後で保菌が持続することがわかっており、1 ~ 2年以上経過すると保菌割合の減少はプラトーに達する(CID2009:48;910)との報告もある。これは MRSA 保菌を持続する病態(糖尿病、呼吸器疾患などの基礎疾患)があるためと考えられ、本論文でも同様の結果であった。これまでは北米や欧州からの報告であり、シンガポールという東南アジアでのデータであり、医療環境の違いはあるものの MRSA の持続保菌期間については大差のない結果であった。また、高齢と長期入院は持続保菌のリスク因子であることも過去の報告と変わらなかった。
