Nail microbial colonization following hand disinfection: a qualitative pilot study

医療従事者による効果的な手指衛生は、院内感染予防の基本原則の 1 つである。本研究の目的は、手指衛生後の爪の微生物定着に関する質的検査の実施であった。結果は、爪の長さ（短い対長い）およびマニキュアの存在（自爪対マニキュア塗布）によって層別化した。潜在的な病原微生物の存在は、爪の長さ（オッズ比［OR］7.1、95％信頼区間［CI］1.83 ～ 27.39、P ＜ 0.001）および紫外線（UV）ライトで硬化したマニキュアの存在（OR 7.2、95％CI 1.25 ～ 40.91、P ＜ 0.05）と相関していた。爪が長い状態であったり、UV ライトで硬化したマニキュアをしているときは、手指衛生が有効でない可能性が高い。

監訳者コメント：
手指衛生において、つけ爪に病原性微生物が付着し、それを媒介としてアウトブレイクになったとの報告はこれまでいくつかの報告がある。一方でマニキュアをつけてもつけなくても、手指衛生に影響がなかったとする報告もある。近年マニキュアのコーティングの方法も増えており、本論文はマニキュアのコーティング方法による手指衛生への影響について検討されたものである。爪が長い（0.2 cm以上を長い）と細菌が指先から検出されやすいことは周知の事実であり、本論文でも同様の結論である。さらに UV ライトで硬化したコーティングで手指衛生が不十分になるのは、コーティングを剥げにくくするために爪を短くする手入れが実施しにくくなり、そのことが手指衛生が不十分になる一因であろうとの考察し、UV ライトを使用するマニキュアは医療従事者には禁止すべきとの結論に至っている。
