Real-world use of fidaxomicin in a large UK tertiary hospital: how effective is it for treating recurrent disease?

研究病院におけるフィダキソマイシン使用の全経過をレビューした。初回再発（6 例）、2 回目の再発（8 例）、3 回目の再発（1 例）にフィダキソマイシンを使用した。1 例が第 1 次選択薬としてフィダキソマイシンを投与された。8 例が初期に治療に反応し、このうち 3 例は 90 日目に無症状、3 例は 30 日目に無症状のまま経過し、2 例は治療中止後 5 日目と 9 日目に再発した。4 例が治療に反応せず、このうち 2 例は糞便微生物移植、1 例は結腸切除術を必要とした。2 例が悪化、2 例が死亡した。フィダキソマイシンに対する忍容性は良好であった。これらの結果から、感染症のこの段階でのフィダキソマイシンの有用性は明らかではないことが示唆される。
