The handwash station: friend or fiend?

手洗いは、交差感染に対する鍵となる防御策で、洗面台（水と排水システム間のインターフェース）で行われる。洗面台について考慮の足りない配置／デザインおよび使用が、幅広くよくみられるが、付随するリスクがないわけではない。文献中で警告されてきたにもかかわらず、2012 年にベルファストで新生児の緑膿菌（Pseudomonas aeruginosa）アウトブレイクが発生して水の管理における変更を余儀なくされるまで、関連する危険の認知について 45 年にもわたり注意が向けられなかった。リスクを最小限に抑えるには、洗面台の蛇口から出る水に対して、緑膿菌やその他の病原体の検査を行うだけにとどまらない全体的なアプローチが求められる。文献によって報告された、新生児病棟以外で洗面台と関連付けられたアウトブレイクは、多剤耐性微生物によるものが多く、カルバペネマーゼ産生微生物によるものが増えつつある。エビデンスによれば、水系伝播の大部分は検出されていないことが示唆されている。洗面台の現行のデザイン、使用および配置を改善するために実行し得ることは非常に多く、このことについて本稿では批判的に評価を行っている。

洗面台について、感染源、感染経路となり得るという視点からのレビューである。環境と感染の関連性については、今後さらにエビデンスが蓄積されているものと思われるが、現段階では判断は難しい。
