成人集中治療室における医療従事者の手指衛生遵守の改善策:小規模なシステマティックレビュー
Strategies to improve hand hygiene compliance among healthcare workers in adult intensive care units: a mini systematic review

背景
集中治療室(ICU)における医療従事者の手指衛生遵守率の低さは懸念すべきものである。
目的
成人 ICU における医療従事者の手指衛生遵守率の向上を図る有効な介入を特定すること。
方法
関連文献について、MeSH term と Text word の組み合わせ(例えば、hand hygiene、hand washing、compliance、adher*、improve*、develop*、intensive care unit)を利用して、OVID Medline および CINAHL の 2 つの主要な電子データベースを検索した。これは Google Scholar および記載の参考文献のハンドサーチにより補完した。次いで、特定した文献のデータを抽出し、品質を評価し、統合効果量に組み入れた。
結果
特定した 89 のタイトルと抄録のうち、最終的に 14 の文献を対象とした。全体的な研究の質は高かった。しかしながら、試験のデザイン・設定・サンプルサイズ・介入のばらつきによってメタアナリシスが不可能であった。したがって、ナラティブ統合を実施した。介入には、教育、観察、用品の提供、利用の改善、指導による支援が含まれ、単独または併用で試験され、1 件を除くすべての研究で良好な成績が得られた。管理支援、「用品」、教育とトレーニング、リマインダー、監査、成績のフィードバックの併用により、遵守率はベースラインの 51.5%から 80.1%に上昇し記録的な値となった。ただし、目的とするほぼ 100%まで遵守率を改善しうる介入の組み合わせはなかった。
結論
入手可能なデータからは、多様な介入が遵守率を「プラトー」レベルまで上げるのに有効であるものの、目的とする基準にまでは達しないということが示唆される。方法論的に適切な複合的介入の試験によって、ICU 職員の手指衛生遵守の改善を目指す介入のエビデンスが強化されるであろう。
