An international survey of cleaning and disinfection practices in the healthcare environment

背景
抗菌薬耐性は、緊急性の高い世界的な健康上の優先事項の 1 つとなっている。基本的な衛生習慣および病院環境の清掃と消毒は、病原体の交差伝播を防ぐ上で重要である。
目的
我々の知る限りでは、医療施設での清掃と消毒の実践に関して世界的な違いを評価した研究はない。本稿に記載した電子的調査は、世界中の医療施設での清掃の実践における違いを評価するために開発された。
方法
International Society of Antimicrobial Chemotherapy（ISAC、旧 ISC）の感染制御ワークグループは、30 の多項選択問題を含む調査を作成した。これらの質問は、世界中の医療現場における現状の清掃の実践内容を評価するために作成された。
結果
オンライン調査には、23 カ国を代表する計 110 名の医療従事者が参加した。96％の施設では書面による清掃方針が存在した。70％の施設では雇用時に清掃スタッフの訓練が行われていた。ただし清掃の実践内容とモニタリングの状況は様々であった。
結論
この調査により、環境の清掃と消毒に対する取り組みに関して、世界中で大幅に異なる実状を認識し、評価することができた。清掃と消毒に関するガイドライン（レコメンデーション）を作成することで、実践内容を改善し、世界的な（ミニマム）スタンダードを設定することができるであろう。

監訳者コメント：
清掃と消毒は、施設間差のみならず、世界的な地域差も大きい。特に清掃については、本邦で受託業者の基準や業務の実施方法は定められているが、清潔の保持の指標となる基準の設定はなされておらず、その取り組みはそれぞれの医療機関に任されてきた。厚生労働科学研究費「標準的な院内清掃のあり方の研究班」では、院内清掃ガイドラインを平成 27 年度に作成、公表した。本論文のような横断的な取り組みと、各国の個別の策定とが有機的に結びつき、清掃分野における進歩と向上が加速することに期待したい。
