携帯電話およびコンピューターのキーボード:3 次集中治療室における多剤耐性菌のリザーバとなる可能性は低い

Mobile phones and computer keyboards: unlikely reservoirs of multidrug-resistant organisms in the tertiary intensive care unit

集中治療室(ICU)の臨床分離株と関連する伝播について検討するために分子疫学的方法を用いた研究は少ない。ICU 患者のルーチンの検体から 13 週にわたって培養された 94 株の多剤耐性病原体(MDRO)を保存した(メチシリン耐性黄色ブドウ球菌[meticillin-resistant Staphylococcus aureus:MRSA]11 株、バンコマイシン耐性腸球菌[vancomycin resistant enterococci:VRE]2 株、グラム陰性菌 81 株)。医療スタッフの個人用携帯電話、ICU の電話、および ICU のキーボードの MDRO を調べるために、スワブ採取し、培養した。2 つの電話から MRSA が分離された。環境および患者の同じ属の分離株を選択し、全ゲノムシークエンシングを実施した。全ゲノムシークエンシングにおいて、携帯電話の分離株では、一塩基多型(SNP)の 2 配列間の距離が 183 であった。しかし、コアゲノムの 15,000 超の SNP より、携帯電話分離株と臨床分離株とが区別された。低流行の環境において、携帯電話やキーボードが院内獲得 MDRO の一因となる可能性は低いようである。

監訳者コメント
MDRO の伝播の原因として、医療環境が注目されている。しかし一方で、全ゲノムシークエンシングのような手技により、菌株の相同性が以前よりも相当詳しく分かるようになっている。実際に電話から MRSA が検出されているわけであるから、携帯電話やキーボードが問題ない、というわけではないと思われるし、臨床検体は研究用に採取された検体ではないので、本研究から何らかの結論を導きだすのは難しいのではないだろうか。
