Needlestick injuries and other occupational exposures to body fluids amongst employees and medical students of a German university: incidence and follow-up

本研究の目的は、血液またはその他の体液への職業曝露の発生、報告、および追跡に関するデータの収集である。職員および医学生（787名）に質問票を配布し、2003年中のその他の体液への職業曝露の公式な報告（203件）と、その後の追跡（100件）を評価した。針刺し・切創の割合は、医学生29.5％、職員22.5％であった。1,000職員・日あたりの発生率は、針刺・鋭利物損傷0.61、体液への皮膚粘膜の曝露0.27であった。過少報告率の平均値は約45％であった。予想に反して、2003年にその他の体液への職業曝露を公式に報告したのは、わずかに看護師4.3％、医師3.9％であった。血清検査不参加者数は追跡期間中に増加した。記録された全検査結果のうち、曝露を受けた100名中35名が来所を取りやめたために追跡不能となった。結論としては、雇用者は安全な器具を提供するとともに、実習形式による教育的介入と事故報告を強化すべきである。医学生と看護学生を含む全職員を対象として、血清検査などの定期的な職場健康診断を義務化すべきである。その他の体液への職業曝露後のコンプライアンスを向上させるために、改良した臨床検査による短期間の追跡について、さらに検討する必要がある。

監訳者コメント：
ドイツの1,400床の大学病院における研究。針刺しなどの職業曝露の報告に関する遵守率を上げることや、その後の追跡が難しいという点を指摘する報告は多い。この点についてはどの国も苦労していると言えよう。安全器具を提供すべきである、教育が必要などといった結論は、他の報告とそれほど異なるものではない。
