chlorhexidine-alcohol in water tanks

最近では、非結核性抗酸菌（NTM）による手術部位感染が、ヒータークーラーユニットの汚染と関連している。現在、欧州疾病予防管理センターと製造会社は、ヒータークーラーユニットタンクを満たす濾過水に過酸化水素を添加することを推奨している。当院においてこの方法を実施したところ、ヒータークーラーユニットは緑膿菌（Pseudomonas aeruginosa）やステノトロホモナス・マルトフィリア（Stenotrophomonas maltophilia）など水中の日和見病原体により高度に汚染されるようになった。NTM は検出されず、急激に増殖した耐性菌がその検出を阻止した可能性がある。過酸化水素とクロルヘキシジン－アルコールの有効性を現場で比較した。微生物の検出効率はおそらく改善されていると考えられたが、クロルヘキシジン－アルコール処理は水中の病原体汚染を阻止し、NTM は検出されなかった。

監訳者コメント：
NTM による汚染が問題になっている体外循環用ヒータークーラーユニットの水の汚染に対する対策に関する論文である。本論文を見る限り、様々な報告が提唱され、まだゴールドスタンダードとなる対策はないようであるが、クロルヘキシジン－アルコール処理は有望な選択肢の一つのようである。
