Optimization of antimicrobial therapy in vancomycin-resistant enterococcal bacteraemia using a rapid detection Gram-positive blood culture assay

迅速分子診断グラム陽性血液培養（BC-GP）アッセイにより、バンコマイシン耐性腸球菌（VRE）血流感染症を迅速に同定することができる。本研究では、VRE 血流感染症患者について、Verigene BC-GP アッセイの導入前（44 例）および導入後（20 例）に評価を行った。VRE 検出までの時間の平均差は、Verigene BC-GP アッセイ導入後の初期では 25.9 ± 4.1 時間（95％信頼区間［CI］ 17.6 ～ 34.1、P ＜ 0.001）であった。Verigene BC-GP 導入前期間の患者と比較して Verigene BC-GP 導入後期間初期の患者では、VRE に対する抗菌薬療法開始までの時間の補正した平均差は 18.2 ± 7.8 時間（95％CI 2.5 ～ 33.8、P ＝ 0.024）であった。

本論文は Verigene BC-GP アッセイの導入により VRE 血流感染症に対する抗菌薬治療開始時間が変化したかどうかを調べている。処方決定に係る 18 時間の短縮は臨床では有意義であると思われた。
