クロストリジウム・ディフィシル(Clostridium difficile)感染症(CDI)は、世界的に主要な医療関連感染症およびアウトブレイクの原因となっている。様々な革新的治療法が開発されてきたが、多面的な感染制御プログラムを用いた予防戦略は CDI 発生率の低減にこれまで成功していない。C. difficile 感染症の主なリスク因子は、抗菌薬の使用によるものであり、一般的には保護作用を有する消化管内の微生物叢の破壊である。特定のプロバイオティクスの補充は、抗菌薬関連下痢症や CDI を含めた様々な有害な転帰を予防する上でこれまでのところ有効である。しかし、どのプロバイオティクス株が CDI を予防し得るかについての合意は得られておらず、様々なプロバイオティクス製品を対象とした研究を統合した場合、メタアナリシスでは高い不均一性が報告されている。我々は、C. difficile 感染症予防における臨床的有効性を評価するために、十分なエビデンスを有するプロバイオティクスに関する文献を検索し、3 つの乳酸菌株(好酸性乳酸桿菌[Lactobacillus acidophilus]CL1285、ラクトバチルス・カゼイ[Lactobacillus casei]LBC80R、ラクトバチルス・ラムノサス[Lactobacillus rhamnosus]CLR2)を配合した特定のプロバイオティック製品(Bio-K+)について、医療環境での CDI 予防における有効性に焦点を当てた。このプロバイオティクス製品に関する文献について、電子データベース(PubMed、Google Scholar)、感染性疾患および感染制御に関する学術会議の抄録、およびプロバイオティック製造企業からの広報を利用して検索を実施した。C. difficile 感染症を抑制する Bio-K+ の作用機序を支持するエビデンスが発見され、優れた安全性および忍容性プロファイルを有することが判明した。Bio-K+ を投与する無作為化対照試験および施設レベルでの介入によるエビデンスは、C. difficile 感染症の発生率が低下することを示している。このプロバイオティクス製品は、抗菌薬投与を受けた患者に投与するならば、医療関連 CDI の一次予防の一翼を担う可能性がある。

ずいぶん「Bio-K+」推しの論文だが、それもそのはず Bio-K+ の会社の人が著者に入っている。もちろん論文執筆にあたっての資金提供も行われている。それらも差し引いた上で、CDI の予防(あるいは治療)におけるプロバイオティクス製品については引き続き興味のあるところである。
